​​​​​​To request a Certificate of Insurance you will need to complete the request form and return to the Risk Management office along with a copy of your contract/and or agreement for the event.

​You Can Request:

     Proof of Insurance: The company is verifying that RSCCD is insured.

     Additional Insured Endorsement: The company is requesting to be insured by the district.
