How to find a Doctor or Dentist

 Anthem Blue Cross HMO or PPO Medical Network


Please visit the Anthem Website  On the right hand side under Resources & Tools there is a link to "Find a doctor".  If you log in as a registered user, the system will pre-populate your medical plan. If you just want to look for doctors without logging in you will need to select the appropriate plan and click the link to find the right provider.

  • Select the type of provider you are looking for
  • Narrow your search to a specialty 
  • Enter zip or city, state 



Delta Dental


If you want to check to see if your dentist is in the network visit the Delta Dental website @ www.deltadentalins.com and click on "Find a Dentist" then select the following:

  • DeltaCare USA (HMO)

Metlife Dental


If you want to check to see if your dentist is in the network visit the Metlife website @ www.metlife.com and click on "Find a Dentist" then select the following:

  • Metlife Dental PPO (PDP Plus)​
